How to use the questionnaires

If you are being asked to fill out the questionnaire

Click on the link to the questionnaire you have been provided with.

Answer the questions to the best of your ability.

Once you are done, click on the "Download Answers" button at the bottom of the page.

Send the downloaded file to securityandcompliance@elixir.com

Repeat for any other questionnaires you have been asked to fill out.

Click on the "Choose Files" button at the top of the page.

Select the file of the answers Elixir provided to you.
